GerOnTe's overall
objective is to improve the quality of life (defined as well-being on 3 levels:
overall health status, physical functioning and social functioning) of
multimorbid elderly patients, while reducing the overall cost of care.
GerOnTe aims to
design a new model of care by bringing together healthcare professionals for
multimorbid patients whose primary disease is cancer.
For clinicians, GerOnTe will
create a single decision centre including:

The
Healthcare Professionals Consortium (HPC)
·
An
Advanced Practice Nurse (APN) as case manager
·
Data
exchange based on information and communication technologies (ICTs), personalised according to patients' needs
For patients and caregivers, GerOnTe:
·
Collect
patient-reported outcome measures (PROMs).
·
Will
coordinate professional care through a single point of contact (= APN nurses).
·
Will
support HPC's key decision-making role through a single application (HolisTM GV), bringing together individual patient data
sources into a common system.
E-Seniors will encourage a proactive engagement of
stakeholders in the project beyond the validation and evaluation phase. This
commitment will be implemented in co-creation sessions (focus groups)
where the target users, older people and their carers,
will be invited to discover the different elements of the GerOnTe
system and to express their requirements and expectations in this respect. Once
the final Holis prototype is ready, small-scale
pilots in real environments will be organised to test
the usability of the system.
The deliverables we are expected to provide for this
project are as follows:

A
coordination strategy for stakeholder engagement related to the creation of the
Holis application
2.
A
practical guide to methods and tools for focus groups (co-creation sessions on
the creation of Holis)
